Job Description

• Seeking an FP/IM with addiction experience provider 
• Work Dates Needed: 3 Month Assignment w/ possible extension. 
• Preferred Schedule: Monday – Friday 8am to 5pm 
• Worksite Setting: Outpatient – Adult
• Scope of Work:  We’re seeking an addiction provider who can see 14 patients per day with an 8am-5pm schedule.  Need to have addiction experience It’s important to understand complex care for patients with multiple chronic medical illnesses and behavioral health concerns.
  
• Req: BC/BE, DEA, BLS, Covid Vaccine
