Job Description

  • Setting: Outpatient clinic for vulnerable populations
  • Dates: ASAP – 6 months
  • Schedule: M-F 8:30a-5p
  • Scope: Complex care for adults with multiple chronic illnesses and behavioral health concerns
  • Duties: Primary care, women’s health, UC, behavioral health, bridging meds
  • Patient population experiences homelessness, poverty, and addiction
  • License & certifications: active CA license, DEA with CA address, BLS 
  • Experience: 2+ years of advanced practice required. Fluency in Spanish is a plus.
  • Pay Rate: $100 per hour
