• Work Dates Needed: ASAP – 3mo with possible extension

• Worksite Setting: 100% outpatient FQHC 
• Scope of Work: 16 patients a day for initial / follow up eye exams 
• Licenses, Certifications, Requirements: CA license, DEA, Board Certified
