- Preferred Schedule: Monday – Thurs 7a – 6p
- Worksite Setting: Clinic
- Scope of Work: Family Practice, Primary, Acute
- On-site dispensing
- Members, spouses, dependents-2yrs and up for acute care, 0-6 months Wellness exams
- Staff and Patient Parking is the same lot, next to building
- No Fees collected – Participates in HomeMed
- Licenses, Certifications, Requirements: CA license, BLS/CPR, highest degree diploma
- EMR: Athena
