Family Nurse Practitioner (FNP-C) needed for Locum Tenens/Any state license

• Work Dates Needed: Feb 17, 2025, or as soon as credentialed and background investigation is completed/approved. Assignment lasts through 8/31/2025 w/option to extend.
• Preferred Schedule: M-F; 8am to 5pm. Although work shifts can vary depending on the need of the clinic. Provider must be willing to be flexible. Provider must rotate into other duty sections as needed to support patient care.
  Worksite Setting: Outpatient Clinic
  
• Scope of Work: Evaluate, diagnose, consult and refer outpatients of all ages. Follow-up, continuity of care and ensuring appropriate forms are completed. Serve on the Patient Centered Medical Home Team
• Licenses, Certifications, Requirements: Credentialing/Background Investigation takes approx. 4 months. Provider must submit a completed/accurate BI packet as soon as possible. Any State APRN/RN license. APRN certification. 3 years of experience. BLS. ACLS. Current immunization record to include COVID, TB Test w/n the last 12 months
• EMR: RPMS/EHR
• Pay Rate Preferred: $90.00-$95.00 plus reasonable travel and lodging
