Job Description
DATES NEEDED Nurse Practitioner (NP)
- Starting as soon as possible, ongoing
POSITION DETAILS Nurse Practitioner (NP)
- Location: Indiana
- Mon-Fri, 8am-5pm
- Setting: VA Clinic
- Will perform Disability Benefit Questionnaires with patients who are Active Duty or Veterans of our Armed Forces
- Patient Volume: Dependent on the number of requests and scheduled patients
- EMR: Web based portal used to fill out the DBQ and submitted to the VA.
- Must be comfortable working independently
- Must be Board Certified, Active IL License
